The form asks for your credit bureau login info, for example, your Experian email and password, and your security question and 4-digit PIN if you have one.
If you don’t have this on hand, just type N/A — we’ll text you a verification code instead.
6. Inquiry Lists (Optional)
If there are specific inquiries you want removed, you can list them here.
And if there are any you don’t want removed, you can list those as well — or we can skip both if you’re not sure.
7. Upload Photo ID
We’ll need a clear, unexpired photo ID — like your driver’s license or state ID.
You can upload it now or send it to me and I’ll attach it for you.
8. Proof of Address
Next, we’ll need a proof of address, such as a recent bank statement or utility bill that shows your name and current address.

10. Tradelines (Optional)
There’s also an option to mark if you’re interested in tradelines to boost your credit profile.

11. Limited Power of Attorney & Authorization
The final step is your electronic signature — it gives us limited power of attorney to communicate with the bureaus on your behalf, strictly for credit services and inquiry deletion.
It doesn’t give full legal control — it just allows us to send and sign dispute letters for you.
You’ll check the box confirming you’ve read and agree to the terms, then sign your name electronically.
12. Marketing Consent
Lastly, you’ll see a box asking for permission to receive text or call updates, including special offers.
You can check that box to stay updated, or skip it if you’d rather not get messages — totally up to you.
13. Final Step
Once that’s all filled out, click Submit Form.
After it’s submitted, I’ll confirm on my end that it came through and we’ll move on to your next step — whether that’s your inquiry count or starting your deletion process.
